Job Summary

We are seeking a dedicated and compassionate Internal Medicine and/or Family Practice Physician to join our primary care team. In this role, you will provide comprehensive primary care services, including well-woman exams, and participate in a rotating after-hours on-call schedule. You will be responsible for managing a panel of patients, emphasizing preventive care, chronic disease management, and promoting healthy lifestyles. This position requires a strong commitment to value-based care, aiming to achieve the best possible patient outcomes while optimizing healthcare resources.

Responsibilities

  • Conduct comprehensive physicals and well-woman exams, including screenings, preventive care, and counseling
  • Deliver high-quality primary care services to a diverse patient population, focusing on preventive care, chronic disease management, and acute care
  • Utilize electronic health record systems using NextGen for data collection and patient management, and to effectively to document patient encounters, treatment plans, and outcomes
  • Develop and implement individualized care plans based on patients' medical history, needs, and goals
  • Participate in quality improvement initiatives and adhere to clinical guidelines and best practices
  • Maintain compliance with all regulatory and accreditation standards
  • Participate in a rotating after-hours on-call schedule to provide continuity of care and address urgent patient needs
  • Stay current with medical advancements, evidence-based practices, and emerging trends in primary care and internal medicine

Skills

  • Medical degree (MD or DO) from an accredited institution
  • Board certification or board eligible in Internal Medicine or Family Practice
  • Valid state medical license and DEA license
  • Proven experience in primary care, with a focus on value-based care models preferred
  • Proficiency in performing well-woman exams and preventive care
  • Excellent interpersonal and communication skills, with a patient-centered approach
  • Ability to work collaboratively in a team-oriented environment
  • Strong organizational and time management skills
  • Commitment to ongoing professional development and continuous learning
