When you are a new patient, you are given a form to update the clinic on your medical history, we do this same process here at Woodlands Diagnostics Clinic. Making sure your medical history is always kept up-to-date can give doctors an essential look into histories or any past diagnoses or current ones and adjust their care to make sure it fits all of your needs. What’s Included In Your Medical History

Essentially, your medical history provides past and present data of:

  • Health condition(s)
  • Surgeries
  • Injuries
  • Diseases
  • Allergies
  • Physical Exams
  • Hospitalizations
  • Accidents
  • Mental Health
  • And additional tests/studies within a medical environment 

Also included in your medical history may be any medication you are currently taking (this can be prescription or over-the-counter) or any medication you may have had a negative reaction to and don’t want to use again. Topics that are in relation to your lifestyle are also brought up in your medical history, this includes stuff like your diet, exercise, smoking or drug use, and/or alcohol. 

Along with the above personal information, you will notice questions also asking about your immediate family’s medical history; this includes past and/or present health information. 

Included in immediate family members are; parents, siblings, children, and grandparents. Although, if an extended family member has sensitive or genetically important conditions that should be noted, this may also be included.
